First, I would like to point out you should receive what you pay for when you go somewhere. The menu for a custom smoothie states that the base will be banana, so I would expect the worker to put banana when making one. I am usually not trying to make a big deal out of things, but I think this is valuable information that they need to remember for next time. This will prevent angry customers wondering why he didn't put any banana in the smoothie in the first place? Secondly, chill on the ice... No one wants to get a smoothie that tastes like ice more than fruit. (Also, why do you need so much ice if you are already putting frozen fruit in it?) However, I gave it two stars because they put a healthy amount of blueberries. About the only fruit they don't skimp you on though.
